Arranging For A Proctored Examination

 

Step One:

Apply for Active Membership.

  • Complete an application form for Active membership online at www.csnm.ca

  • Include the following:

    • copy of diploma

    • copy of transcript of marks

    • $40.00 non-refundable application processing fee

Mail your application package to:

Step Two:

The application package will be reviewed by the Membership Committee for approval to write the exam.

Once approved for the exam, an email will be sent advising you of the approval.

 
Step Three:

The approval email will contain three important links - one for the Rules and Guidelines, for a list of the Proctor Qualifications, and the last for a Proctor Package (All of these documents are on our website).

A deadline date to submit the Examination Fees and the completed proctor package will be given and enforced. Late submissions will forfeit their opportunity to write the exam on the scheduled date. An exam candidate is allowed ONE deferral, and if they choose to use that deferral will be moved to the next available exam date. (ONLY ONE deferral will be allowed)

The exam candidate is responsible to find and secure a suitable proctor. The list of the Proctor Qualifications is on the website.

Contact a person qualifying as a suitable proctor and ask him or her to proctor your examination on the date specified in the accompanying letter from 9:00 am. to 12:00 p.m. local time. If the person agrees to the specific date and time, arrange a place for the examination. The exam candidate must pay all expenses associated with proctoring the examinations.

Complete the information required in the ENTRANCE EXAM AND PROCTOR REGISTRATION PACKAGE, and send it along with the current the Entrance Examination Fee to the CSNM Office. The package and payment should be received in the office on or prior to the specified date in the accompanying letter.

You can submit payment and send in documentation by one of the following methods:

  • Pay the exam fee online, then scan & email documentation to the CSNM Office at csnm@csnm.ca

  • Pay the exam fee online, then fax documentation to the CSNM Office to Fax number: 416-441-0591

  • Mail into the CSNM Office. Please allow three to five days for mailing. The mailing address is as follows:

The Examination Portfolio Manager reserves the right to disapprove the proposal based upon his or her assessment of the request. You will be notified by CSNM National Office if the proposed proctor is not approved.

Upon approval, the examination(s) and instructions will be sent to the proctor. It is the exam candidate’s responsibility to confirm the date of the scheduled examination, time of 9:00 am. to 12:00 p.m. local time and the location with the proctor.

The exam candidate is to notify the CSNM Office, the Examination Portfolio Manager and Proctor in writing, if you are unable to sit for the scheduled entrance examination a minimum of 48 hours before the exam is scheduled.

The exam candidate must also notify the CSNM office immediately csnm@csnm.caif there has been a change in the approved proctor.
